Apply
for a Library Card Online
To obtain a
card, fill out the application below completely. There is additional
private information that will be needed and your card will not be
useable until you visit the library, present proof of address and photo
identification, and sign your card. Patrons under the age of
18 must have a parent (with photo identification) sign for their card
before it is useable.
The Blackfoot Public Library has reciprocal agreements with the
following libraries:
